Welcome Bag Request Form

This form is to request a Welcome Bag for a child impacted by the foster care system in the Tri-County area. Please complete a SEPARATE form for each child you are requesting a Welcome Bag for. Please be detailed and complete all questions, as we pack each bag unique for each child. 

Before completing the form below, please make sure you meet the following criteria: 

  1. You live in Peoria, Tazewell, or Woodford counties (or your case is through Peoria, Tazewell, or Woodford counties) 
  2. You: 
  • have accepted a new foster placement within the last 30 days OR
  • have accepted a new kinship/relative placement within the last 30 days OR 
  • are a biological family reunified within the last 30 days 
  • are a biological family working an intact case plan 
  • are a teen in foster care working an independence goal 
As much as we want to help caregivers be prepared to take a placement, we cannot approve any requests or deliver welcome bags until placement has occurred. To help us with this, please do not submit this form until the child has been officially placed in your home.

We'll reach out if we have any questions and to let you know when to expect your Welcome Bag(s) to be delivered.

This form is designed to be completed by the caregiver for the child(ren) in their home. This helps to ensure accurate needs and clothing sizes are provided, as well as contact information for the family receiving the Welcome Bag(s).

If you are a caseworker or someone else completing the form on behalf of another person, please provide the family's contact information below in addition to yours (already provided above). 

AGENCY/CASE INFORMATION

The below agency and/or caseworker information is required to ensure the validity of all Welcome Bag requests. Foster Village Peoria will reach out to the agency and/or worker information provided to confirm requests as needed. If information is not provided, we will follow-up to request that information from you. If you have not yet been assigned a caseworker, please note that below and instead provide the name of someone else you have been working with (licensing, investigator, etc.)

Please provide first and last name, if able. If you can provide their title or department (I.e. caseworker, licensing, investigator, etc.) that is helpful as well!

Either email address or cell phone number is required. Providing both is helpful, if you're able!

Please detail the COUNTY your case is out of. We typically serve families with foster care cases in Peoria, Tazewell or Woodford counties.

Ex: loves sports, favorite color is purple, emotional/mental age, any sensory needs, etc.

DEMOGRAPHIC INFORMATION

Our non-profit is funded solely by donations and grants. For grant purposes, we are required to obtain the following demographic information. Information about the child's race/ethnicity also helps our team provide appropriate hair care products for each child.

Please list the size # as well as whether it's a Toddler / Child / Adult size.

Additional Items

Occasionally, we have additional items in our Resource Closet that are not a part of our regular Welcome Bag packing list. Please note: These additional items are NOT guaranteed to be included. If we have additional items available in stock that meet your child's needs/size/gender, we will see if we can provide that as well. This is not guaranteed to be included and depends on our current inventory availability.

NEXT STEPS

We aim to pack and deliver all Welcome Bags to you within 24-48 hours of receiving your request. We understand that some situations are emergent and items are needed sooner than others.

Upon receiving your request(s), our Resource Closet Coordinator will review your request and assign a volunteer to pack and deliver it to you.

We try to deliver as soon as we can, but sometimes due to volunteer availability delivery can take up to 48 hours. We will reach out to let you know when to anticipate the delivery to your home. If you have questions in the meantime, feel free to reach out to us at katie@fostervillagepeoria.org.
